Health Form Reminder for PreK, K, 6th Graders and New Students

This is a reminder for all PreK, Kindergarten, 6th Grade, and new students to the district. We need to have your health forms on file in the nurse's office if you have not turned them in yet. If you have any questions, please contact Nurse Caldwell at tcaldwell@sd171.org. She will be happy to answer your questions. All forms are posted on the district website on the Enrollment page in both English and Spanish.
